When you register as a patient, you consent to InterHealth Medical Clinic's doctors and staff accessing and using your personal information to provide the best possible healthcare. Only authorized staff will access your information. Additional consent will be obtained if needed for other purposes.
Why Is My Personal Information Collected, Used, Held, and Shared?

Your doctor collects personal information to manage your health and provide medical services. It is also used for business activities like financial claims, practice audits, and staff training.

Records are securely stored electronically or in protected physical formats. Access is restricted to authorized staff. Doctors and staff sign confidentiality agreements and adhere to strict password policies.​​

How Can You Access and Correct Your Personal Information?
​
You may request access to or correction of your medical records by completing a Request for Personal Health Information form. Requests are typically processed within 14 days, with a fee of $0.20 per page plus a $42.02 administration fee.
